Under the direction of the Community Program Manager, the Community Support Worker will provide supports for assigned caseload participating in Inclusion Services in order to develop and support Person-Centered job opportunities/community skills/ engagement enhance home living skills; address health and safety as needed and other duties as assigned to support individuals served for adults with intellectual developmental disabilities.The Community Support Worker will maintain or improve interaction skills in the community or at the person's place of employment. Community Support Worker will assist with retention of independent living skills such as shopping, banking, money management and use of public transportation assistance with social interaction skills and community involvement. The Community Support Worker will network with employers and people in the community to create viable job/volunteer opportunities, assist individuals with ADL's health related supports, meal preparation, household services and money management for each person served, to obtain and maintain employment in order to increase economic independence, self-reliance, social connections and the ability to grow within a career.The Community Support Worker will participate as part of the Interdisciplinary Team to ensure that the individual's success in attaining and maintaining meaningful community employment and community engagement, enhance the person's ability to live independently in the community as specified in the ISP and associated supports plan. Community Support Worker will model training and support strategies for community employers, assist to develop natural supports and advocate for the person to be integrated into the work/community culture. Documents progress on outcomes/action plans daily.

Salary : Up to $18.00 per hour
