This position is responsible for coordinating the care delivery interventions for residents in their assigned area in accordance with current professional practice standards, physician's orders, facility policies and procedures, and local, state and federal regulations and within the scope of practice. Must have knowledge of PDPM. Open to Licensed Practical Nurse and Registered Nurse. Minimal On-Call.
